Consent Information

Understand the service, how your information will be used and what you are agreeing to before you proceed.

This page explains the general consent approach for services operated by Dr. Govind Rathod through drgovindrathod.com and app.drgovindrathod.com. Service-specific information and the applicable privacy notice must be provided before consent is requested.

Reading this page, browsing the website or making a payment is not, by itself, consent to every use of your health information or to every form of care.

Consent Information

1. Services covered

This information applies to the free 12-question quiz, ₹499 Health Assessment, Doctor Review, 90-Day Improvement Program and Annual Membership. Occupational Health and corporate services require additional information and consent arrangements appropriate to the service and the employee’s situation. A corporate contract does not replace individual consent where it is required.

Consent Information

2. Your decision

Consent means agreeing voluntarily after receiving an understandable explanation of the relevant service, its purpose, expected benefits, important limitations, foreseeable risks and alternatives. You may ask questions, decline a service or decline an optional use of your information. Consent to one service is not blanket consent to unrelated services, marketing or research. Where care cannot be provided safely without certain information or permission, this will be explained before you decide.

Consent Information

3. Eligibility and identity

Individual clinical services are for adults aged 18 or older located in India, booking for themselves. Before clinical care, your identity, age, contact details and current location may need to be verified in a way appropriate to the service. Provide accurate information. Where age, identity or capacity to consent is uncertain, further verification or another care arrangement may be needed.

Consent Information

4. How consent is obtained

Consent may be requested through an unticked electronic checkbox, a signed form, a written confirmation or another appropriate documented method. The method used for your service will be explained. The consent record should identify the patient, the relevant service and permissions, the date and time, and the wording or version used. Where verbal consent is appropriate, it should be documented in the clinical record. A website information page does not replace this process.

Consent Information

5. Free quiz and automated results

The quiz provides general awareness and next-step guidance based on your answers and automated clinical rules. It does not provide an individually physician-reviewed diagnosis or a validated percentage probability of disease. All 12 required questions must be completed. Before you submit answers, the quiz should explain what information is processed and whether anything is retained, including relevant technical logs. Do not assume that a downloadable report means a copy will remain available later. Download and retain your own report if you wish.

Consent Information

6. Assessment and medical consultation

The ₹499 assessment includes physician review and sign-off of the report; it does not include the advanced consultation package. Doctor Review includes consultations and a signed written report within its stated scope. Programs and membership combine medical review with coaching within the purchased service. Before clinical care, the doctor should explain the proposed assessment and consultation, its limitations, relevant alternatives and whether an examination or referral may be needed. Separate consent may be needed for a new procedure, materially different intervention or other activity beyond the original agreement.

Consent Information

7. Remote-care limitations

Consultations may use video, phone or written review according to the service and clinical situation. Remote care cannot always replace an in-person examination. Technical problems, incomplete information or the nature of a condition may limit assessment. The doctor may recommend in-person care, additional evaluation or referral. Consent does not guarantee a diagnosis, prescription, specific improvement, disease reversal or medication reduction. Continue existing prescribed treatment unless a physician advises a change. These services are not an emergency service; seek urgent medical care for urgent concerns.

Consent Information

8. Health information and privacy

Depending on the service, information may include your identity and contact details, health history, medicines, quiz answers, reports, BP, weight, waist, glucose and other measurements, consultation notes, care plans and consent records. Only information relevant to the stated purpose should be requested. Before collection, the applicable privacy notice must explain the purpose, the submission and storage arrangements, who receives or accesses the information, the retention/deletion approach and how to raise concerns. This page does not make a claim about a particular storage provider, hosting location, retention period or security certification. Read the current Privacy Policy and the service-specific notice; ask for clarification if a material detail is missing.

Consent Information

9. Doctor, coach and administrative access

The clinical team may include Dr. Govind Rathod and authorised coaches supporting the agreed program. Relevant coach access must be disclosed as part of the care and data-use explanation before consent is taken. Access should be limited to what each person needs for their role. Coaches support the agreed plan; they cannot independently diagnose, prescribe or change medicines. Administrative staff should receive only the information needed for administration. Any technology-provider access or other disclosure must be explained in the applicable privacy notice. Unrelated uses require an appropriate separate basis or permission.

Consent Information

10. Email, WhatsApp and calls

Service communication may use email, WhatsApp and calls, with a designated route for health-data submission where provided. Required channels will be explained before enrolment; not every channel may be interchangeable. Prescriptions, when clinically appropriate, are sent by email with your permission. Confirm the recipient address and number and tell us if they change. Email and messaging can expose information through shared devices, notifications, forwarding, incorrect addresses or third-party systems. If you cannot accept a required channel, discuss this before booking so we can explain available arrangements and any effect on service delivery. Permission to communicate does not waive confidentiality or other applicable responsibilities.

Consent Information

11. Recording consultations

Routine clinical notes and documentation of consent are different from audio or video recordings. If recording a consultation is proposed, its purpose, access, retention and any available alternative must be explained, and separate permission obtained before recording starts. This page does not authorise recording by the team or the patient. Ask and obtain appropriate agreement before making or sharing a recording.

Consent Information

12. Refusal and withdrawal

You may decline a proposed service or tell the doctor that you do not wish to continue a consultation or proposed intervention. To withdraw consent for a specified future use of your information, send a written request to contact@drgovindrathod.com identifying the permission you wish to withdraw; do not include unnecessary medical records. Your identity may need verification. Withdrawal does not undo care already provided or processing already lawfully completed. It may prevent further delivery of a service that needs that information or permission. It does not automatically create a refund or require deletion of records that must lawfully be retained. Refunds are considered under the applicable policy and your legal rights. Any continued retention or processing needs an appropriate lawful basis.

Consent Information

13. Optional uses

Patient reports, images, stories or recordings are not authorised for testimonials, marketing, teaching or research merely because you agree to care. If such a use is proposed, you will be contacted and asked for separate, specific, optional consent explaining the intended use and disclosure. Refusing an optional use does not affect access to the agreed care. Any applicable research approvals and additional requirements must also be met.

Consent Information

14. Occupational Health and corporate services

Before an individual assessment, explain its purpose, any employment-related or statutory context, the intended recipients and what information may be reported to an employer. Distinguish a fitness conclusion, attendance information or aggregate findings from detailed medical results. Do not assume an employer is entitled to every clinical detail. Individual confidentiality, applicable reporting duties and any relevant limits to consent must be explained in the service-specific notice. This general page does not authorise unspecified disclosure of employee medical records.

Consent Information

15. Understanding and questions

The written page is in English. An explanation can be requested in English, Hindi or Gujarati, and you should have an opportunity to ask questions before agreeing. If you do not understand an explanation, say so before proceeding. You may request the relevant service information and a copy of your documented consent, subject to appropriate identity verification.

Consent Information

16. Related policies and updates

Consent information should be read together with the service description, Terms, Privacy Policy, Medical Disclaimer and Refunds and Cancellation Policy. Fees and cancellation rules do not remove applicable consent or patient rights. This page is effective from 20 October 2026. Material changes to the purposes or scope of an agreement should be explained, with fresh consent obtained where required.
