Sample Report · See what you receive

See the structure.
Understand the difference.

Explore how a free quiz snapshot, a physician-signed assessment and a Doctor Review report help answer different questions.

Look for the information considered, the limits, what needs attention and the next step—not just a page of numbers.

The 12-question quiz is compulsory before either paid assessment route. It provides automated awareness guidance, not a diagnosis.

Read the samples correctly

Examples of the deliverable—not findings about you.

These on-page previews explain the structure and purpose of each output. The free preview follows the supplied quiz sample’s format; the paid previews illustrate the agreed service scope.

They contain no patient measurements, personal identifiers, signatures, prescriptions or calculated health scores. They are not complete patient reports, and the paid examples are not evidence that a clinical review has already occurred.

Cardio-Metabolic Health Snapshot

Preliminary awareness output · generated automatically from entered information

Health at a glance

A summary of the entered information, an automated priority and which data were available.

Your numbers

Reported measurements such as BP, weight, waist and glucose information, with units where available.

Health domains

The areas covered by the available answers, with unassessed areas identified.

What stands out
A summary of findings selected by the quiz rules. An automated priority needs clinical context; a doctor may identify a different priority.
Your health story and why it matters
An explanation connecting the reported information within the quiz’s limits.
Focus first and other areas to keep in view
An automated first focus alongside additional observations. This is not a treatment plan.
What to do next
Next-step guidance, including appropriate doctor review where needed.
Clinical note
The output is preliminary and does not replace appropriate clinical evaluation.
Questionnaire completion and clinical data availability are different. Completing the quiz does not mean every health domain has been assessed. Missing information must not be read as a normal result.

The supplied example specifically identifies lipids as not assessed. Its inclusion here demonstrates the importance of stating limits, rather than claiming complete coverage from questionnaire completion alone.

The preview does not reproduce the sample’s values, priority labels or thresholds as universal clinical rules. Your actual output depends on the answers and information supplied.

3. First priority
The main finding or question identified for attention using clinical judgment.
4. Recommended next step
A suitable next action or clinical discussion based on the available information.
5. Suggested reassessment
What may need reviewing again, with appropriate timing or context.
Actual deliverable: physician-reviewed, verified and signed PDF in English. This illustrative preview has no signature or patient-specific advice.

This adds individual physician interpretation to the automated starting point. It does not include a one-to-one treatment consultation, prescribing or medication changes.

The signed PDF is delivered within 24 hours of payment verification and complete required intake. Older reports may be accepted, but interpretation reflects their dates rather than establishing current health from old information.

Common questions

Know what the sample does—and does not—show.

Is this a diagnosis or a report about me?

No. These are illustrative deliverable previews. Your own output depends on your information and, for paid services, the agreed review.

Will a doctor review the free quiz result?

No individual physician review is included in the free quiz. The ₹499 assessment and Doctor Review have their own physician-reviewed scope.

Must I buy ₹499 before Doctor Review?

No. Complete the compulsory quiz, then choose the appropriate paid route. The ₹499 assessment is not a mandatory preceding purchase.

Can I download these samples?

This page provides on-page previews. It does not publish patient-report downloads. Your actual service deliverable is a downloadable PDF through the instructions provided for that service.

Where do I send my records?

Ask for the designated clinical submission route and applicable privacy/consent information before sharing records. Do not attach medical records to an initial administrative enquiry.

Should I order all advanced tests to prepare?

No. The doctor advises the required panel and preparation for your case. A sample report is not a test order.
